    Many of you are fortunate to be employed by a company which provides dental benefits for yourself and your family. In many cases, these benefits do not cover 100% of all dentistry that is required. We will do our best to help you understand your benefit coverage, but we also ask for your help with the following:

    • Please get current information from your dental benefits carrier before scheduling appointments. You may have been given a wallet-sized card including your group and identification numbers as well as a benefits booklet. Please bring these items with you to your first appointment and/or whenever your employer, benefits carrier or coverage changes or you receive a new benefits booklet
    • Benefit coverage is a contract between yourself, the insurance company and your employer or union, not the dentist. We act as a provider of dental services and have no say about your coverage
    • Dental benefit carriers do not inform dental offices of changes in your coverage
    • We will process your dental claims, in most cases electronically, so that reimbursement is usually very quick
    • Most benefit carriers will only speak to the patient directly about questions regarding their dental claims, and will not give our office any information
    • The treatment plan that you receive from Dr. Crossman is based on a very thorough examination of your teeth, and your dental plan may not cover the complete costs of the specific treatment that you require. We will inform you in advance of the costs and schedule appointments that are comfortable for you financially
    • The fees that we charge are from the current Ontario Dental Association fee guide, which is updated each calendar year
    • We would be happy to complete a preauthorizaton to send to your dental benefits carrier for confirmation of your coverage. This will help you to plan your finances before we begin treatment
    • Please feel free to ask any questions you may have regarding fees and payments, and we will do our utmost to help you
